Plant and Refinery Explosions are Preventable

Every plant and refinery explosion shares something in common: they could (and should) have been prevented.

As Ryan Zehl discussed with the Houma Times, plant owners and operators are financially incentivized to focus on profits and deadlines over the safety of workers and contractors.

And when profits becomes more important than safety in a chemical plant filled with hazardous and highly combustible liquids, gases and hydrocarbons, the consequences are inevitably going to be catastrophic.

Unfortunately, this trend only appears to be getting worse.  Between 2018 and 2019, for example, there were more plant and refinery explosions in Texas and Louisiana than in the previous 5 years combined.

Some of the most common causes of these Plant and Refinery Explosions included:

– Flammable Liquids and Gases

Chemical plants contain volatile hydrocarbons, liquids and gases that, if exposed to heat or other ignition sources, instantly ignite.

There are a number of safety processes and procedures that, when followed, prevent any ignition sources from being introduced in areas where flammable hydrocarbons, liquids or vapors may be present, including Process Hazard Analysis, Job Safety Analysis, Lockout/Tagout Procedures, and the use of “intrinsically” safe equipment.

– Combustible Dust

Combustible dust is commonly identified as a cause of plant and refinery explosions.

Federal and state safety regulations require plant owners and operators to ensure that combustible dust is safely stored and isolated so that it doesn’t accumulate on or around machinery, equipment, or in any other area where it could be exposed to heat or potential ignition sources.

– Hot Work

Sparks from hot work, including welding, torching, brazing, heating, and soldering, can easily travel 35 feet or farther.

Hot work permits, Lockout/Tagout procedures, work and gas handling plans, and other permitting procedures must be utilized and followed to ensure that hot work does not occur anywhere where flammable gases, liquids or vapors may be present.

– The Use of Equipment and Machinery that is Not “Intrinsically Safe”

OSHA regulations prohibit equipment and machinery that is not classified as “intrinsically safe” for use in hydrocarbon atmospheres from being used in plants and refineries where combustible gases, liquids or vapors may be present.

Unfortunately, plant owners and operators commonly ignore these regulations, leading to catastrophic fires and explosions that could and should have been prevented.
